    How to Send Your Guardianship Nomination Form to the Secretary of State

    Several years ago, the Nevada Legislature passed our bill into law that would allow a person to nominate their own Guardian should the need for them to have one ever arise.

    Once the law was official and the form designed, approved and was ready for use, we met with the Secretary’s staff and were instructed to make sure that we did not copy or print the form double-faced, but make them single faced and submit the two pages that way.

    In each of our seminars and speaking engagements we have stressed that “two-page instruction rule and all has been well to date, with more than 2,000 forms filed thus far.
    Several weeks ago, we were told that a resident had mailed his form in, as instructed, two pages, however they had been returned with instructions to send it back double-faced.

    To make sure that this return had not been in error, I contacted the Secretary’s office, and the following is what I received today, 4/25/2025;

    Apologies for my delayed response, Rana. For notarization purposes, we strongly prefer that Guardianship forms be single-sided. While we understand that double-sided forms may occasionally be submitted, and we will process them, we kindly request that senders prioritize single-sided printing. The volume of double-sided forms received is low, so we will not reject them.

    Additionally, please let us know if it would be helpful to talk with Ruben Rodriguez, our Deputy Secretary for Southern Nevada, who oversees this program. He’s happy to do an interview with the Vegas Voice to get more information out there if that would be helpful.

    Thank you,

    We will be reaching out to Mr. Rodriquez for an interview and will arrange for him to speak with SCA residents who might like to clarify any questions they may have. Once that is confirmed we will post information giving residents the date and time that an informational meeting will take place
